Because our church exists to serve and glorify Jesus Christ as Savior, we feel it is necessary that persons involved in this position of rearing our covenant children be personally committed to Jesus Christ. Please give a brief testimony of your Christian Experience including how you came to know Jesus Christ as your Savior and how you maintain a close relationship with Him. (This is for non-Members as Members have been previously examined on this issue.) If you prefer to give this information in person, please set up a time to share with the Children's Ministry Director. You may put days and times you are available in the testimony section below.

To ensure the safety of our children, volunteers must be realistically certain that they would not abuse the children in their care. Volunteers must also be free from substance abuse.

Consent for Criminal Background Check

I hereby give my permission to Faith Presbyterian Church, Gainesville, FL to obtain information relating to my criminal history record. The criminal history record may include arrest and conviction data as well as plea bargains and deferred adjudications and delinquent conduct committed as a juvenile. I understand that this information will be used, in part, to determine my eligibility for an employment/volunteer position with this organization. I also understand that the criminal background check will be performed within six months of signing this form. I understand that, if necessary, I will have an opportunity to review the criminal history as received by Faith Presbyterian Church, Gainesville, FL and a procedure is available for clarification, if I dispute the record as received. I also understand that the criminal history could contain information presumed to be expunged. I also understand that as this check is related to the children’s ministry that any records could preclude me/remove me from working with children until all has been cleared.
